You must obtain a written telemedicine referral from a DEA-authorized provider who has conducted an in-person evaluation prior to establishing or continuing medical care with Dr. David Scheiner; if you are taking a schedule II controlled substance (ex. Adderall, Ritalin, Vyvanse, Concerta, Focalin, Aztarys, Daytrana, methylphenidate, amphetamine-dextroamphetamine etc.) or schedule III-V narcotic (ex. Codeine, Tramadol (Ultram) etc.).  The referral must contain the provider’s name and national provider identifier (NPI) number.
